Who can call the ASHLine?
The ASHLine provides services to help people at all stages of quitting tobacco use. This includes those who are ready to quit, thinking about quitting, or who have quit and want help staying tobacco-free.
The ASHLine also provides information on the quitting process for family and friends who want to learn more about the quitting process.
ASHLine quit coaching and quit kits are free and available to all Arizona residents. Arizona residents can call the ASHLine anytime to learn more about our services and enrollment options.

Is Nicotine Replacement Therapy (NRT) available?
Eight weeks of nicotine replacement therapy (NRT) including nicotine patches, nicotine gum, and nicotine lozenges is now available to participants. Combination therapy with two products is included!
Participants using only the web-based ASHLine Program are eligible for two weeks of NRT.
Participants who choose the telephone-based Quit Coaching Program are eligible for an additional six weeks of NRT.
NRT is available to all Arizona residents 18 years or older and medically eligible.
Enhanced text messaging will provide participants with additional support and resources, including features such as text to order NRT, and a short code text to engage in the text message program.
What is our confidentiality policy?
The ASHLine takes privacy very seriously. All of the information ASHLine collects is used only for reporting, evaluating, and advancing the evidence for smoking cessation.

Make a Referral to ASHLine
Providers and community partners can refer individuals to ASHLine in several ways, depending on workflow and system access. Referrals may be submitted through a secure online provider web referral form, by fax using the ASHLine Fax Form, or via eReferral through participating electronic health record systems. All referral methods require basic participant information and consent to be contacted. HIPAA-covered entities may receive progress updates, while non-HIPAA-covered organizations can still submit referrals to connect individuals to quit support.
The online provider web referral form is the fastest and easiest way to connect individuals to ASHLine services. The secure form allows providers and community partners to submit referrals electronically by entering participant contact information and consent to be contacted. Once submitted, ASHLine will reach out directly to the individual to offer quit coaching and available services. HIPAA-covered entities may receive progress updates based on participant status.

ASHLine Referral Fax Form: A referral form for providers and community organizations to securely connect individuals to ASHLine services. The fax form allows providers to request outreach, document consent, and authorize nicotine replacement therapy when applicable. Includes informed consent in both English and Spanish languages.

ASHLine Menthol Brochure (English & Spanish): An overview of menthol tobacco and nicotine use and how ASHLine supports individuals who use menthol products. The brochure explains why menthol can make quitting more difficult and highlights free, specialized quit coaching, tools, and incentives available through the ASHLine Menthol Tobacco Program. ASHLine Quit Plan (English & Spanish): A guided tool that helps individuals create a personalized plan to quit tobacco or nicotine. The quit plan supports goal-setting, identifying triggers, building support, and using effective strategies. Available in both English and Spanish languages.
